The Committee for Scientific Research Ethics, College of Veterinary Medicine / University of Al-Qadisiyah, announces the adoption of the Research Ethics Application Form as an editable and electronically signable PDF. Applicants are required to follow the instructions below:
Download the Research Ethics Application Form in PDF format.
The form must be completed exclusively by the research supervisor or the Corresponding Author.
Complete all required fields, including the relevant Human Studies or Animal Studies section, as applicable.
Complete the electronic signature in the designated field and save the completed application as a PDF after verifying all information.
The applicant must provide their official university/institutional email address in the form. Applications containing personal or non-official email addresses will be rejected.
The completed application must be submitted exclusively from the same official email address recorded for the applicant in the form. Applications submitted from a different or non-official email address will not be accepted.
